I acknowledge that participation in training at The Grid Training involves inherent risks, including the risk of injury, and I voluntarily assume all risks on behalf of the minor participant named above. I release The Grid Training, Joshua Pelzl, and staff from liability for injury or loss arising from participation, except to the extent caused by gross negligence. I confirm the participant is physically able to take part in physical activity, that I have disclosed any relevant medical conditions or allergies above, and that I will inform Coach Josh of any change in health. As parent or legal guardian, I am authorized to consent to this minor's participation and I agree to these terms on their behalf.
